18-21 Transition Programming

Transition planning is the process of preparing your student to move from high school into the adult community. This planning involves collaboration among the student, their family, and school staff to create a plan that addresses the student's individual needs.

Cozad Community Schools Transition Program offer services for secondary students with disabilities, ages 18 - 21, who have either completed their high school program with a certificate of completion or have not yet received their high school diploma. The services provided are based on the recommendations of the IEP team, tailored to address each student's specific transition needs.

Instruction in the Transition Program is customized to help students achieve their transition goals in the following areas:

  • Vocational Development

  • Independent Living Skills

  • Community Participation

  • Recreation and Leisure

  • Post-Secondary Education or Training

Related services are provided as determined by each student's IEP team during their transition IEP meeting, held in the spring of their senior year. This meeting includes the high school IEP team, a transition teacher or special education supervisor, and any involved agencies. Together, the IEP team creates a transition plan based on the student's individual needs and personal preferences.
