We are pleased to announce that Two Rivers Public Health Department, in collaboration with Cozad Community Schools, will be making influenza vaccinations available to the Cozad students and staff this fall.
To sign up for an Influenza vaccination, please complete and return the consent form – one individual per registration form – to the school office by Friday, October 25th. Please be sure to sign and date the form and attach a copy of front and back of insurance card. The health department staff will be at your schools on Wednesday, October 30th to give immunizations.
