Transition planning is the process of preparing students as they move from high school into the adult community. This planning involves collaboration among the student, their family, and school staff to create a plan that addresses the student's individual needs.
Students will have meaningful and inclusive opportunities to develop awareness, understanding, and exploration of post-secondary options in employment, education, independent living, and community involvement. These opportunities will be grounded in:

Appropriate resources and tools
Authentic experiences
These student-centered opportunities will prepare individuals to pursue learning, earning, and living by advocating for themselves, contributing to their communities, and actively engaging in society.
